Our One Wellness Medical Clinics are participating providers in the Chronic Disease Management Programme (CDMP), established to improve the level of care for chronic disease patients and make treatment more accessible and affordable for Singaporeans.
CDMP covers these 23 conditions:
- Diabetes Mellitus / Pre-Diabetes
- Hypertension
- Lipid disorders
- Stroke
- Anxiety
- Bipolar Disorder
- Major Depression
- Schizophrenia
- Asthma
- Chronic Obstructive Pulmonary Disease (COPD)
- Allergic Rhinitis
- Parkinson’s Disease
- Osteoarthritis
- Nephrosis/ Nephritis
- Benign Prostatic Hyperplasia
- Dementia
- Epilepsy
- Osteoporosis

Medisave 500/700 Scheme
Per annum, a patient can use up to $500 for non-complex conditions or $700 for complex chronic conditions, under the MediSave 500/700 Scheme. Each MediSave claim is subject to a 15% co-payment in cash.
From 1 February 2024, residents enrolled in the Chronic Disease Management Programme (CDMP) with chronic conditions can use their MediSave to fully cover the cost of treatment at their enrolled HealthierSG clinics, up to the MediSave500/700 withdrawal limit. Patients no longer need to co-pay 15% of their bill in cash.
Claim up to $700 for complex chronic conditions yearly.
Patients with chronic conditions under the CDMP can also tap on government subsidies of up to $500 per year under the Community Health Assist Scheme (CHAS) if they are CHAS cardholders, and special subsidies of up to $540 per year if they are Pioneer Generation or Merdeka Generation cardholders.
